This process works on assigning appropriate codes against the diagnosis, procedures, supplies and tests as mentioned in charts received from hospitals. Review clinical documentation and diagnostic results as appropriate (i.e., to extract data and apply appropriate ICD-9-CM/CPT-4 codes for billing, internal and external reporting, research, and regulatory compliance). Under the direction of Health Information Management, must be able to accurately code conditions and procedures for complex Inpatient and Surgery accounts or code two or more outpatient patient types at an expert level, per the Coding Guidelines.
Essential Duties and Responsibilities
Thoroughly reviews all documenation in the medical record and assigns codes for all diagnoses, treatments, and procedures according to the appropriate classification system for inpatient/outpatient encounters.
Reviews physician assigned diagnosis code after thorough review of the medical record and, if necessary, queries physician for additional clarity in a professional manner. Thorough knowlege of modifiers, HCPCS, denial handlling. Must be able to demonstrate exceptional Inpatient coding & Surgery coding proficiency.
Able to accurately abstract information from the medial records into the abstract system, according to established guidelines.
Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and adheres to all official coding guidelines.
Follows all policies and procedures set forth by Accretive and the assigned client; including operational procedures for placing accounts on hold, querying, deficiencies, completing accounts, etc.
Consistently meets all productivity and quality metrics
Typical Qualifications
CPC, CPC-H or CCS
Must be able to demonstrate proficiency in Inpatient & Surgery coding type with 95%-99% accuracy.
4-6 years of Inpatient and Surgery coding experience, preferred.
Must be able to use standard office equipment and information systems
Ability to interact with other employees through effective communication.
Ability to prioritize and shift workloads.
